Event Cancelations

In line with the current restrictions in place at Middlesex Health, the Center for Chronic Care Management has canceled several upcoming events:

    • Wellbeing Series talks scheduled for March & April

We appreciate your understanding as we do everything we can to keep our patients, staff and community safe. We look forward to seeing you at future events.

Managing a chronic illness like heart failure, asthma, or diabetes can be difficult. At Middlesex Health, we care about helping you manage your chronic illness successfully. We know that the smarter approach requires care beyond the walls of the hospital or doctor's office. That’s why the Center for Chronic Care Management offers programs that help patients with chronic illnesses to live well with their conditions. We offer one-on-one counseling to give you the opportunity to learn more about your condition, to get your questions answered, and to help you put together a care plan.

Since 1999, the Center's dedicated and specialized professional staff of nurses, counselors, dietitians and care managers has helped thousands of people here in Connecticut enjoy better lives and improved self health management.

Am I eligible for one of these programs?

To be eligible, patients must have a physician affiliated with Middlesex Health. If they do not already have a physician, the Center staff will assist patients in selecting one. All care managers work in cooperation with the patient’s physician to provide education and guidance to manage their chronic conditions. Patients with multiple diagnoses may be enrolled in more than one program.
