COVID-19 >>New Vaccination Information

  • Vaccine supply continues to be limited, so Middlesex Health is scheduling first dose appointments based on the number of doses we receive each week.  We are currently offering appointments for:

    • Health Care Personnel
    • Long-Term Care Facility Residents
    • Medical First Responders
    • Individuals 55+ years of age
    • Pre-K-12 Teachers, paraprofessionals, custodial staff, food service providers, school bus drivers and childcare providers as well as in-school administrative staff. Does not include individuals who are not required to work on-site in a school.

    Please note that we will require proof of eligibility, such as an employee ID, pay stub, letter from employer, or business card to verify employment, and/or a goverment issued ID to verify date of birth.
