COVID-19 Vaccine for Minors

Beginning May 13, 2021 the COVID-19 vaccine can be administered to individuals aged 12+. There are, however, some important things you need to know if you are under 18 years old, or the parent or legal guardian of someone who is.

Proof of Age

The FDA has approved the Pfizer vaccine for individuals aged 12+. You MUST provide a government-issued proof of age before you receive the vaccine. Eligible documents include a valid birth certificate, driver's license or passport.

Parent or Guardian Must Attend

Those under the age of 18 cannot legally give their consent to be given the vaccine, so a parent or legal guardian must accompany the minor to their appointment.  The parent or legal guardian will need to sign the "Consent to Treat" form before we are able to administer the vaccine.

Pfizer Only

Individuals who are under 18 MUST receive the Pfizer vaccine as this is the only one approved for use for those under 18 years of age.
