Anticoagulation Clinic

The Middlesex Health Anticoagulation Clinic is an outpatient service dedicated to managing patients on warfarin. We use a point-of-care device to obtain immediate International Normalized Ratio (INR) results using a small sample of blood from a finger prick.  We are then able to adjust your warfarin doses based on the INR and other factors such as changes in diet, medications, or health.  Our team includes a medical director, nurse practitioner, clinical pharmacists, and administrative assistants. 

Referral Process

To become a patient at our clinic, ask the doctor who prescribed you warfarin to fill out this Anti-Coagulation Clinic Form and fax it to our office. Once we have the appropriate paperwork, we will contact you to schedule your first appointment.
