When

There are three main times when you can enroll for benefits:

As a new hire – usually, within the first 30 days from your hire date.

Each year, Open Enrollment is usually in late October/early November.

Qualifying life events – such as marriage, divorce, birth, adoption, or a change in employment status. With a qualifying life event, please notify benefits@midhosp.org to advise them of any changes to your elections within 31 days of the event.

Where

Complete your benefits enrollment in Infor Employee Space, then submit it to Human Resources/Benefits.

Your benefits will become effective on the first of the month, 30 days from your hire date. For example, if you were hired on October 12, your insurance coverage would start on December 1.

You can view your current elected benefits in Infor Employee Space.

No, your decisions will apply for the following benefits year, which runs from January 1 through December 31. You must review your benefit elections and circumstances for each open Enrollment to ensure you are covered appropriately for the year ahead.

However, you can make changes if you experience a qualifying life event (such as marriage, divorce, birth, adoption, or a change in employment status). With a qualifying life event, you must declare the event and make any associated changes to your elections within 31 days of the event.

Carrier information can be found on the relevant benefit page. For example, the Dental page contains Information and the contact details for the Dental carrier Delta Dental.

If you leave Middlesex Health, your benefits end on the last date of employment or contracted hours. If you are retiring, your benefits end on the previous day of the month you retire.
