It is safe to come to our offices when necessary—but to keep you and your family as safe and healthy as possible, we are recommending that your appointment
move from in-person to virtual.

 

Step 1: Contact Us

Changing your appointment from an in-person visit to a virtual visit is simple—just call your provider's office, and our staff will help you update your appointment. You will be able to keep the time and date of your original appointment.

Step 2: Get Set Up for Your Visit

Your provider's office will let you know which platform will be used for your virtual visit.

The visit will either take place through the MyChart patient portal or over Zoom. Either way, we have many resources available to make sure you are ready for your appointment.

It only takes a few steps to sign up online for a MyChart account at Middlesex Health!

  1. Request an Activation Code
    Click the button below to get started. You will be asked to provide some basic information about yourself, and we will send you an Activation Code.

  2. Complete the Sign Up Process
    Click the button below to go to the MyChart Sign Up screen. Enter your activation code and date of birth, then follow the prompts to complete the sign up process.

Virtual Visit Instructions

Our MyChart support team is available to help with technical trouble with your MyChart account. If you are not able to use your activation code, can't log in or are having other similar issues, please contact us.
