Current Vaccine Eligibility

Per state and federal guidelines, individuals six months or older are currently eligible to receive a COVID-19 vaccine.

Getting Started & Vaccine Information 

Both the FDA and CDC agree that it is safe and effective for individuals to choose an updated vaccine that is a different vaccine type than they originally received. You will have the ability to choose your vaccine type when you book your appointment.

It is recommended that you DO NOT get a COVID-19 vaccine if you are allergic to any ingredient in the vaccine. Consider talking to your primary care physician or other provider before getting the vaccine if:

  • You have a history of anaphylaxis or severe allergy to any substance (foods, including shellfish; medication; etc.)
  • You have a condition that significantly compromises your immune system
  • You are pregnant or breastfeeding
  • You have a bleeding disorder (including low platelet count) or are on a blood thinner
  • You have any questions about other specific health care concerns

Proof of Age

You MUST provide a government-issued proof of age for your child before they can receive the vaccine. Eligible documents include a valid birth certificate or passport.

Parent or Guardian Must Attend

Those under the age of 18 cannot legally give their consent to be given the vaccine, so a parent or legal guardian must accompany the minor to their appointment. The parent or legal guardian will need to sign the "Consent to Treat" form before we are able to administer the vaccine.

After the Vaccine

If you experience side effects and believe you need medical attention, please contact your health care provider. Always dial 911 or go to your nearest emergency department in case of a medical emergency.

Please note that your provider is required to report some side effects to the CDC.

All Middlesex Health patients will be asked if they've been vaccinated for COVID-19. If you've been vaccinated, but not by Middlesex Health, you may be asked for proof of vaccination.

If you are a Middlesex health patient, A record that you received the COVID-19 vaccine will be added to Epic, our secure electronic medical records system.

You can view this record in the MyChart patient portal, in the Immunizations section under Health Summary.
