For Our Pediatric Patients

Well-child visits are vital to monitor your child’s growth and development. Middlesex Health Family Medicine and our team of board-certified pediatricians are here for you and your child.
We will provide head-to-toe physical exams and evaluate their development to ensure they are on track. Well-child visits also offer the opportunity to discuss any questions or concerns you have regarding your child’s health.
We offer an online patient paperwork portal for our pediatric patients. On this page, you will find a packet of paperwork customized to your child's age. Please follow the instructions to complete this packet before your appointment. You can email it to us ahead of time or bring a printed copy to your/your child's appointment.

Videoconference with your health care providers from the safety & comfort of your own home.
As we work together to navigate the COVID-19 pandemic, Middlesex Health is now offering Virtual Visits—also called telehealth or telemedicine visits—with providers. This will allow you to have certain types of appointments using your MyChart account on your phone, tablet, or computer.
Middlesex Health Family Medicine offers many different types of virtual appointments, including new patient visits, sick visits, follow-ups, care management visits and Medicare Wellness Visits.

What is a Patient Centered Medical Home?
A Patient Centered Medical Home (PCMH) is a primary care practice that combines teamwork and technology to provide evidence-based care that meets your unique health care needs and goals.
What does that mean for me?
At a PCMH, you are the most important member of your health care team. You and your providers will work together to build a positive and productive long-term relationship. It also means that your primary care office is coordinating with other health care providers you might see. This leads to better care, a more positive experience and lower costs for you and your family.
How does a practice become a Patient Centered Medical Home?
A PCMH designation must be earned. The National Committee for Quality Assurance (NCQA) only recognizes a practice as a PCMH if they meet rigorous standards for high-quality, coordinated, patient-centered care.
Middlesex Health Primary Care has achieved Level 3 recognition—the highest given by the NCQA.

The MyChart patient portal creates a single medical and financial record through which you can:

Preparing For Your Visit
We understand that your time is precious, and our goal is to make sure that you get the most out of your visit with us. Please consider this short "to do" list—taking a few simple steps can help us ensure that we meet all of your expectations.
- Make a List of Your Questions & Concerns
Writing questions or concerns out ahead of time helps you and your doctor plan your visit. Plus, it reduces the chance of forgetting to mention something important. - Prioritize Your Concerns
Ask us your most pressing questions first! This ensures that we have plenty of time to fully address what you are most concerned about, as there are some time constraints on your visit with us. - Bring a Friend or Family Member
It is always helpful to have a second set of ears! A trusted support person will help you remember what we discussed and make sure that we don't miss anything important. - Bring Your Medication Bottles
In order to have the most productive conversations about your medications, it is essential that your entire care team knows exactly what you take. - Bring Other Helpful Medical Information
For example, if you are diabetic and you know your provider will ask about your sugar levels, bring your glucometer. If you have high blood pressure, please bring your list BP readings since your last visit.
