When You Arrive
The Middlesex Health Westbrook Vaccine Clinic is located at 250 Flat Rock Pl, Westbrook, CT 06498. When you arrive enter through the entrance that is marked with signs. You will then fill out an attestation form and proceed to the second floor where you can follow signs to the vaccination clinic.
