The Chronic Heart Failure Care Program assists individuals with Chronic Heart Failure and their caregivers to manage symptoms, improve overall health, achieve a more active lifestyle and reduce the need for emergency room visits. The care manager works with you and your doctor to design a plan to meet your needs.

One on one counseling services are available for:

  • Symptom control
  • Making an emergency plan
  • Planning diet changes
  • Understanding your medications
  • Making a physical activity plan 
  • Answering questions about your diagnosis
