Vaccine supply continues to be limited, so Middlesex Health is scheduling first dose appointments based on the number of doses we receive each week. We are currently offering appointments for:
Health Care Personnel
Long-Term Care Facility Residents
Medical First Responders
Individuals 55+ years of age
Pre-K-12 Teachers, paraprofessionals, custodial staff, food service providers, school bus drivers and childcare providers as well as in-school administrative staff. Does not include individuals who are not required to work on-site in a school.

Please note that we will require proof of eligibility, such as an employee ID, pay stub, letter from employer, or business card to verify employment, and/or a goverment issued ID to verify date of birth.
Please check your insurance coverage to be sure you and your physician have met the required pre-treatment authorizations (i.e., second opinions or pre-certification of services.) If these conditions are not met, your insurance company could deny benefits and you will be responsible for payment.
For most scheduled services, our Registration Department will contact you in advance to obtain the required information about you and your insurance. We can then have the forms ready for your signature on the day of your appointment.
Please visit the Registration department before going to your appointment and bring your insurance card. You may also contact our Billing Office.
Your Bills
Hospital Bills Middlesex Health bills your insurance company if you provide us with the proper information. If insurance payment is delayed, or there is a balance due after insurance payment, a statement will be mailed to you. Read the message carefully as action may be required on your part.
Physician's Bills Your private physician will bill you separately for their services. If you have questions about these bills, please call the phone number printed on their statement.
Medicare Patients
Please bring your Medicare card with you. Middlesex Health will bill Medicare directly if you furnish us with the proper information. You will receive an Explanation of Benefits from Medicare indicating what Medicare has paid.
No Insurance/Patient Balance after Insurance Payment
Payment is due in full unless a payment plan is authorized. We accept checks and credit card payments. For your convenience, you may pay your bill online.
Financial Assistance
You may be eligible for financial assistance for all or part of your balance due if your total family income is within the guidelines. To inquire, please call our Financial Counselor at 860-358-2402 or 860-358-2403. Our staff will be glad to work with you to help you understand your insurance coverage.
